神経障害性痛に対する新しい外科的治療法に関する研究

Research Abstract

再生する末梢神経が中枢に及ぼす影響とメカニズムを解析する実験を行った。針刺し事故に近い大型動物実験モデルとして、ビーグル犬の前腕の尺骨神経と正中神経を18G針で刺して、神経障害性の疼痛モデルとした。この神経の回復過程を電気生理学的に解析した。末梢から刺激を与えたとき、障害を与えた中枢での神経活動電位を測定することによって神経障害性疼痛のメカニズムに迫ることが必要と考え、そのシステムを確立した。刺激と測定の両方に微小タングステン電極を用いて、日本光電製Neuropackで感覚神経の電位を計測して、その治療過程を記録した。また、脊髄刺激によるSEPの測定を行い、局所の末梢神経の障害がSEPにどのような影響を与えるのか評価した。犬の個体による差が大きいものの、局所に生じたNeuromaが病理組織学的に大きい個体ほど神経障害が大きいことが判明した。このことは局所を人工神経で再生させることにより病態を改善させられる可能性を示唆するものとして、臨床上有意義であると考えられた。
