細胞内カルシウム放出制御による新しい悪性高熱症・不整脈の治療開発と臨床応用

Research Abstract

最近、我々は、悪性高熱症(MH)の治療薬であるダントロレンは、リアノジン受容体(RyR)のN-terminal domainに結合し、ドメイン連関障害を是正することによりRyRからのCa2+漏出を抑制するという新しい分子作用機序を報告した(Kobayashi et al, J Biol Chem.2005;280:6580-7)。さらに、イヌ心不全モデルやCPVT型knock-in マウスにおいても、ダントロレンは、不全心筋の異常リアノジン受容体を分子標的とし、心筋細胞機能を改善させ、新しい心不全・不整脈治療薬になる可能性があることを最近報告した(J Am Coll Cardiol. 2009; 53:1993-2005, Circ J.2010;74:2579-84)。申請者らは平成23年から、ヒト心不全患者のダントロレンの心室頻拍に対する有効性と安全性を検討するための臨床試験(YUMIN4346)を開始した。平成23年度~25年度で基礎心疾患を有する心機能の低下した心室頻拍の3症例に対して、ダントロレンの臨床試験を行い、ダントロレンの安全性および抗不整脈効果を有することを確認した。ダントロレンの投与方法(内服、静注)の検討や副作用のチェック、その効果について検討した。ダントロレンは、心抑制がなく、従来の抗不整脈薬や心不全治療薬とは全く異なった作用機序(異常リアノジン受容体を分子標的)を持つことから、心機能の低下した心不全患者や薬物抵抗性の致死的不整脈に対する新しい治療薬となることが期待された。
