| Research Abstract |
心不全患者において睡眠時無呼吸の有無を検討すると共に、血管内皮機能に及ぼす低酸素の影響を検討し、睡眠障害を改善するための看護ケアプログラムの構築を目指した。研究分担者(安藤眞一)が併任している福岡県済生会二日市病院の協力も得て症例を蓄積し検討会にて研究結果のフィードバックを行いながら遅滞なく進行するように努めた。
１）心不全患者における睡眠障害の検討
安定した心不全患者に対し下肢加温による補完的治療の有効性を検討した。脳性ナトリウム利尿ポリペプチドや心臓超音波による左室駆出率などを心機能の指標とし、終夜睡眠ポリグラフィ(PSG)を用いた客観的評価に加えて睡眠質問票により主観的にも睡眠を評価した。また、17名の睡眠障害を合併する心不全患者において下肢加温ケアの前後でPSGの連続気流記録と指尖の血中酸素飽和度曲線から心機能を推定する指標(ラグタイム)を考案した。これは無呼吸後の呼吸再開時点から指尖の酸素飽和度が上昇するまでの時間を酸素化された血液が指先まで届く時間：ラグタイムとして自動計算したものである。下肢加温により睡眠が改善した心不全患者においてはこのラグタイムも改善しており簡便な心機能の指標になることが示唆された。看護ケア上も有用な指標であった。
２）睡眠障害患者における低酸素と血流依存性血管拡張反応検査(FMD検査)の検討
32人の睡眠呼吸障害患者に終夜ポリソムノグラフィとFMD検査を行い低酸素状態の頻度や最低酸素飽和度値ではなく蓄積的酸素飽和度の程度が心機能やFMDを低下させることが分かった。この事より睡眠呼吸障害においては蓄積的低酸素状態を改善することが心機能低下の予防となる事が示唆された。3年間の挑戦的萌芽研究により心不全と睡眠呼吸障害の間で悪循環をきたすメカニズムについて考察する所見を得ることができた。また、このような科学的根拠を看護ケアに生かすことが今後の課題である。
