| Outline of Annual Research Achievements |
本研究は、再発・難治性B細胞性非ホジキンリンパ腫を対象に、CD19抗原を認識するキメラ抗原受容体（CAR: chimeric antigen receptor）発現Tリンパ球を用いる養子免疫遺伝子療法を開発することを目的とした。研究成果は次の通りである。1) CAR発現Tリンパ球の抗腫瘍効果増強を目的に、CARの他にIL-21を発現させた。IL-21共発現型Tリンパ球では、IL-21産生と下流シグナルのSTAT3のリン酸化亢進を認めた。２）抗原特異的IFN-γ産生やヒトBリンパ腫細胞株に対する殺細胞効果は、IL-21発現による増強効果を認めなかった。3）担癌免疫不全マウスへの遺伝子改変Tリンパ球の投与実験では、腫瘍増殖抑制効果が認められたが、CAR単独とIL-21共発現型Tリンパ球の治療効果に有意な差を認めなかった。４）抗腫瘍サイトカインなどを併用した場合の安全対策として、腫瘍局所での発現を可能にする分子スイッチ（転写因子NF-AT誘導型プロモーター）を作製した。レポーター遺伝子を用いた解析から、本スイッチを導入したCAR発現Tリンパ球では、CARシグナル特異的な遺伝子発現制御が可能であることが試験管内および担癌マウスモデルで明らかになった。5）臨床研究と同様な方法で遺伝子改変Tリンパ球の調整を施設内細胞プロセシング室で行った。患者（同意済み）Tリンパ球へのCD19特異的CAR遺伝子の導入と体外増幅が可能であることと、それらのBリンパ腫細胞株に対する細胞傷害活性を試験管内で確認した。
再発・難治性B細胞性非ホジキンリンパ腫に対する養子免疫遺伝子療法の臨床研究については、遺伝子治療臨床研究「CD19抗原特異的キメラ抗原受容体発現Tリンパ球を用いた難治性B細胞性悪性リンパ腫に対する遺伝子治療臨床研究」が厚労省で承認された。臨床研究を開始し、患者登録を行っている。
