高エネルギー陽子ビームを用いた次世代型粒子線治療のための基盤研究

Outline of Annual Research Achievements

高精度の陽子線治療と病院併設型加速器中性子捕捉療法(BNCT)を行うことのできる次世代型治療装置を開発中であり，本治療により線量分布の良い陽子線により腫瘍の物理学的ターゲッテイングが可能となり，がん細胞致死効果の高いBNCTにより放射線抵抗性，浸潤性，多発性の克服といった腫瘍の生物学的ターゲッテイングが可能となる．本研究では，高エネルギー陽子ビームを用いた次世代型粒子線治療のための基盤研究として，以下の４つのテーマを実行し，基礎と臨床をつなぐトランスレーショナルリサーチを行った．Ⅰ．陽子線およびBNCTによる高次の抗腫瘍効果発現メカニズムの解明については，低線量粒子線照射によるマウス小腸および精巣のアポトーシスを検討し，0.05Gy以下の低線量でも線量依存性があることが確認された．Ⅱ．ホウ素化合物の腫瘍内輸送法の開発と細胞内分布の解明については，PETによる解析を行うための基盤整備を行った．Ⅲ．陽子線とBNCTの最適な併用法の探索に関する検討については，マウス悪性黒色腫の局所照射後にサイトカインが誘発され、とくにIFN-γの上昇は局所効果と関連がある可能性が示唆された．また，化学療法と粒子線治療の併用効果について，基礎的な研究を開始した．Ⅳ．新しい臨床試験の立案と実行についえは，小児腫瘍に対する陽子線治療の安全性，有効性評価を約130例に行い長期的な経過観察を開始している．進行非小細胞肺癌に対する化学療法併用陽子線療法については試験を終了し，国際誌に掲載された．また新たに，BNCTの臨床試験を学内の倫理委員会に申請し，原子炉を利用した臨床研究であるが，4例の治療を終了し経過観察を開始している．
