家族性高コレステロール血症の新規原因遺伝子が臨床像と生命予後に与える影響

PCSK9は細胞内コレステロールプールの現象によりSREBP2活性化を介し誘導され、LDL受容体の細胞内分解を促進すると考えられている。したがって、その機能亢進型変異は家族性高コレステロール血症（FH）の原因となり、機能低下型変異は家族性低βリポ蛋白血症の原因となる。われわれが見出したPCSK9 E32K変異は一般人の1.7%に存在すると考えられる高頻度変異である。
文書による同意取得後、FHが疑われる症例の末梢血を用いHRM法、MPLA法にて既知のLDL受容体遺伝子変異を同定した。一方、LDL受容体変異を認めない症例に対し、genomic DNAを抽出しPCSK9遺伝子全エクソンに対するプライマーを設定し、直接塩基配列法にて遺伝子変異を決定した。LDL受容体変異による家族性高コレステロール血症321例を対象にPCSK9遺伝子変異のスクリーニングを行った。
最終的に昨年までに見出した6例のほかにLDL受容体遺伝子異常とPCSK9 E32K変異のダブルヘテロ接合体を見出せなかった。さらにPCSK9 V4I変異もわれわれのコーホート中には見出されなかった。PCSK9遺伝子に新規変異を認めた場合に、遺伝子導入ゼブラフィッシュを樹立し、その機能をin vivoで検討する予定であったが、新規遺伝子の発見に至らなかった。また、その前検討として既知のPCSK9遺伝子E32K変異導入ゼブラフィッシュの作成を試みたが、成功に至らなかった。
PCSK9遺伝子の家族性高コレステロール血症における意義は、高頻度変異E32Kで説明される部分が大きいと考えられる。
