久山町高齢者におけるＭＲＩを基盤にした脳血管障害、認知症、うつ病の疫学研究

Research Abstract

①認知症、うつ病、ADL、脳血管障害の調査：　平成24年から25年にかけて福岡県久山町に在住の65歳以上の高齢者を対象とした高齢者調査を実施し、当該年齢人口の93％に相当する1904名が調査に参加した。一次調査では、認知症の評価として長谷川式簡易知能評価スケール改訂版(HDSR)、Mini-Mental State Examination(MMSE)、Clinical Dementia Rating (CDR)、うつ病の評価としてCenter for Epidemiologic Studies for Depression State (CES-D)、ADLの評価としてBarthel Indexを、それぞれ質問紙を用いて調査した。さらに、認知症やうつ病が疑われた住民に対しては二次調査を実施し、専門医による診察や家族・主治医との面接、臨床情報の収集を行い、確定診断を行った。また、脳血管障害の既往や発症が疑われた住民についても診察や臨床情報の収集を行い、診断を確定させた。
②頭部MRI検査：　高齢者調査の対象者のうち1326名に対して施行した頭部MRI画像撮影について、脳卒中を専門とする複数の医師が読影し、主要病変（脳梗塞、脳出血、白質病変、主幹動脈狭窄、脳動脈瘤、脳腫瘍など）の有無や部位、大きさについて評価した。さらに、voxel-based morphometry（VBM）の手法を用いて、頭蓋内容積、脳容積、灰白質容積、白質容積を算出し、また海馬など脳の部位別の容積を測定した。
③データベースの整備：　上記の検査・調査から得られたデータを整理し、横断研究や追跡調査に用いるデータベースの整備に着手した。

Reason

久山町の65歳以上の高齢者のうち1904名が調査に参加し、93％という高い受診率を達成することができた。そのうち1326名については頭部MRIの画像情報が得られ、脳の主要病変の有無について評価することができた。

Strategy for Future Research Activity

高齢者調査および頭部MRI検査によって得られた情報を用いて、統計解析に用いるデータベースを完成させる。さらに、脳の形態学的所見と各疾患（認知症、うつ病、ADL障害、脳血管障害）との関係を、他の臨床データの影響を考慮しながら横断的・縦断的に検討する。研究計画の変更や研究遂行上の問題点は特にない。
