Outline of Annual Research Achievements

我々は先行研究において、血圧表現型、殊に高血圧関連遺伝子との関連は、家庭血圧（HBP）によって精度高く行い得た（Hypertens Res 2010;33:129, Hypertens Res 2012;35:1102）。
そこで高血圧発症、降圧反応性と遺伝子の関連を知る前提とし、平成25年度、平成26年度において、本科学研究費に依り、大迫研究において、HBP、自由行動下血圧（ABP）と臓器障害、予後の関連に関する多数の成果報告を行った。（BMJ 2013; 1報, Hypertension 2013; 4報, J Hypertens 2013; 3報, Am J Hypertens 2013; 11報, Clin Exp Hypertens 2013; 2報, Hypertens Res 2013; 1報, Int J Cardiol 2013; 1報, 他）
高血圧の成因と治療に関する遺伝子解析として、HBPを用いたRCTであるHOMED-BP研究において第一選択薬であるアンジオテンシンⅡ拮抗薬（ARB）、カルシウム拮抗薬（CCB）、A-Ⅰ変換酵素阻害薬（ACE-I）の降圧作用に関連したSNPsをGWASにより網羅的に解析したHOMED-BP-GENE研究の成果を公表した。（Pharmacogenomics 2013; 14:1709）
その結果降圧効果に関連のあるいくつかのSNPsが各薬剤において複数見出された。また、高齢者を対象とした長期縦断研究（SONIC）において、ARBにおけるABCC9rs1283807、CCBにおけるrs588076遺伝子多型との関連が確認された。これら遺伝子型を臨床に速やかに反映させることは高血圧のテーラーメード治療の可能性を示唆するものであり、本科学研究費による大きな成果であると考えられる。

Reason

研究実績の概要に記したように、平成25年度、26年度において、本科研費の研究テーマに沿った家庭血圧（HBP）と高血圧の成因、降圧薬の作用に関する研究成績は順調に報告されている。また、最も重要なテーマである遺伝子とHBPに基づく降圧効果との関連に関する研究は、HOMED-BP-GENE研究の成果がPharmacogenomics 2013;14:1709に報告されたことから、おおむね順調に目標を達成していると考える。更に平成25年度から26年度に一部予算を繰り越し、この間大迫研究、HOMED-BP研究、J-HOME研究などの観察、介入試験より、未治療本態性高血圧患者にアンジオテンシンⅡ受容体拮抗薬（カンデサルタン、アジルサルタン、あるいはイルベサルタン）を単独で投与し、HBPにより降圧効果を評価した257人において、インフォームドコンセント取得の上DNA採取が終了し、平成26年末よりGWASによって我々が明らかにした。降圧反応性遺伝子の網羅的解析が開始されており、平成27年度にはその分析結果が提示しうる所に到っていることから、研究の達成度はおおむね順調といえる。

Strategy for Future Research Activity

平成25年に終了の予定であった遺伝子とHBPによる降圧薬の降圧作用評価の関連を分析するためのDNA検体採取は平成25年度の予算繰越し申請に記した理由で遅れた。平成26年度にこの遅れを取り戻した。平成26年12月までに257検体のDNAの採取が終了し、平成27年1月よりHBPによるアンジオテンシンⅡ受容体拮抗薬（ARB）の降圧効果評価と降圧反応性遺伝子との関連分析が開始されている。これは、平成25年度に成果として出されたHOMED-BP-GENE研究のreplication研究であると同時に、ARB降圧反応性遺伝子の新たなる探索でもある。本分析は、平成27年度の中頃には終え、その成績を高血圧学会、循環器病学会等に公表、論文化の予定である。
