Outline of Annual Research Achievements

研究代表者はキルギス共和国 (キルギス)からの要請により、2006年に医学交流のため同国を訪問した際に、我が国では人口10万人当たり0.5人以下の発症率と現在ではほとんど診ることのなくなったリウマチ熱 (Rheumatic fever: RF)が中央アジア諸国では未だに猛威を奮っていることを見出した。さらに、キルギス国立心臓病センターとの国際共同研究により顕在化しないRFの後遺症も含めたリウマチ性心疾患 (Rheumatic heart diseases：RHD)が国土の94 %以上が海抜千メートル以上の山地から成り、山岳地帯ともなれば 4000メートルを超える高地での生活、つまり低酸素と、肉食中心の食習慣とソビエト時代からのウオッカの飲酒とあわせて、当該地域での死因の過半数を占める60歳までの心不全の潜在的リスクファクターであることを報告した。本研究のもと、キルギスのほぼ全土を網羅する2000名を超える詳細なフィールド調査を行い、RF/RHDの有無、聴打診、心電図、心エコーなどの理学的所見、溶連菌を中心とした細菌感染症の頻度、バイオマス燃料、食習慣、遊牧民のテントであるユルタに住んでいるか否か等の生活歴、畜産の有無、慢性呼吸器疾患などの既往歴、家族歴など100を超える項目に関する調査を完遂した。これらの項目を解析した結果、以下のデータを得た。
(1)溶連菌だけでなく肺炎球菌、インフルエンザb 型菌などの呼吸器感染症を引き起こす細菌との混合感染が有意に認められること。
(2)暖房・調理などの際にいわゆるバイオマス燃料を用いることで、粉塵・COなどが、35分間の家事でタバコ 5本分に相当する粘膜免疫に対するリスクとなること
(3) 溶連菌感染によるRF/RHDの発症頻度(日本に比べ、約45倍高い)と集積度に関する検討から宿主因子が存在する可能性
