Outline of Annual Research Achievements

慢性心房細動に対しては,心房内の双極電極カテーテルで記録される分裂電位CFAE(complex fractionated atrial electrogram)を標的としたカテーテルアブレーションが行われてきたが,臨床や動物実験における最新のマッピング装置を用いても,心房細動の慢性化やCFAEとの関連が疑われる因子を一つずつ調べることは不可能なため,その成因には不明な点が多い.CFAE標的アブレーションの有用性についても研究者間でばらつきがある.本研究はそのような混沌とした慢性心房細動の治療戦略に理論的根拠を与えるため,ヒト慢性心房細動のコンピュータモデルを開発し,CFAE成因をシミュレーション実験で検証した上で,慢性心房細動アブレーションにおける最適戦略の提案を目指すものである.
研究代表者らは,これまでに心房筋において増生した線維芽細胞と心房筋細胞との電気的結合が,心房筋の電気生理学的性質を変化させ,興奮伝播遅延・ブロックを生じさせることで心房細動の慢性化をもたらすこと,さらにはそれがアブレーション治療に有効なCFAE成因になりうること等をシミュレーションで示してきた.本研究は,さらにそれを推し進めるものであり,これまでに,線維芽細胞の増生による心房筋の電気生理学的かつ構造的な変化が,心房細動の基本成因とされるローター(スパイラルリエントリー)の定在性と安定性を変化させ,心房筋における興奮波増減バランスを決定付けること,さらにはカテーテルアブレーションによる心房細動治療の成否は,従来から考えられていたような異常自動能の除去やリエントリー回路の遮断によるものではなく,ローターの制御による興奮波増減バランスの変化によるものであること等を示唆することができた.また,これを臨床で検証するため,本研究成果をヒト心房細動のリアルタイムマッピング装置の開発に応用した.
