リンパ球造血異常の解析からの骨髄増殖性腫瘍の病態解明へのアプローチ

Outline of Annual Research Achievements

造血器とリンパ組織の腫瘍のWHO分類2008において骨髄増殖性腫瘍の一つとして分類されている「好酸球増加とPDGFRA、PDGFRBまたはFGFR1遺伝子の異常をともなう骨髄系とリンパ系腫瘍」では、これまでPDGFRAと融合する遺伝子は6種類が報告されていたが、我々は第7番目の融合遺伝子FOXP1を同定した。FOXP1-PDGFRAの融合遺伝子のmRNAとゲノムDNAの塩基配列を比較したところ、FOXP1遺伝子の本来転写されるべきコドン鎖ではなく、アンチコドン鎖が逆方向に翻訳されているという現象を見出した。骨髄増殖性腫瘍である本態性血小板血症 (ET)ではJAK2、CALR、MPLの遺伝子変異が報告されているが、66例のCALRの遺伝子変異を伴うETからその臨床的特徴を解析した。CALR変異を伴うETでは、血小板数はJAK2変異を伴うETのそれより高値を示したものの、血栓症の頻度は低かった。また、type 2 CALR変異と染色体異常を伴った症例が二次性骨髄線維症や急性巨核芽球性白血病へ移行したことから、type 2 CALR変異と染色体異常が疾患の進行に関与する可能性が示された (20th Congress of the EHAで発表)。60歳以下の急性骨髄性白血病症例の白血病細胞におけるサイトカイン受容体であるIL-3受容体alpha鎖、GM-CSF受容体alpha鎖、IL-2受容体apha鎖、common gamma鎖、c-kit、FLT3、G-CSF受容体の発現を検討し、予後との関連を検討したところ、IL-2受容体apha鎖の発現が予後不良因子であり、その発現レベルにより3群に分けて検討したところ、発現レベルにより予後が異なり、発現レベルが高いもの程予後が不良であったことを報告した。
