薬剤耐性アスペルギルスの耐性機序解明と病原因子制御による新治療戦略の開発

Outline of Annual Research Achievements

本研究は、アスペルギルスの薬剤耐性機序の解明と耐性獲得阻止法の開発、ならびに病原因子の制御による新治療戦略の開発を行うことを目的としている。
①アゾール系抗真菌薬感受性株、耐性株の病原性確認とアゾール系抗真菌薬に対するin vivoでの感受性検討、②in vivo、in vitroにおける耐性化誘導能に関する検討、③耐性化の機序解明と耐性化抑制に関する検討、④マクロライド系薬による病原因子制御の可能性について研究を行った。
①アゾール系抗真菌薬耐性株、感受性株を用いて、侵襲性肺アスペルギルス感染マウスモデルに感染させ、イトラコナゾール(ITCZ)を種々の濃度で吸入させて治療した場合、低濃度では感受性株で耐性株に比較してマウスの生存率が向上したが、高濃度の吸入では、耐性株も感受性株と同様に、マウスの生存率が向上した。マウス肺内でのITCZの濃度を測定した結果、高濃度吸入した場合にITCZの最小発育阻止濃度を上回る濃度に到達することが確認できた。アゾール系抗真菌薬の吸入療法の有用性を証明した。
②③慢性肺アスペルギルス症マウスモデルについて、非結核性抗酸菌を先行感染させ、その後にアスペルギルスを感染させる共感染モデルを作成した。非結核性抗酸菌症が先行すると、アスペルギルスがより長期間感染することが確認された。in vitroにおいて、低濃度のアゾール系抗真菌薬をアスペルギルスに暴露し続けると薬剤に対する感受性が低下した。アゾール系抗真菌薬の標的部位であるCYP51Aの変異は認められず、他の耐性機序の存在が明らかとなった。
④in vitroで気管支上皮細胞であるH292細胞を用いて、アスペルギルスのムチンMUC5AC産生能とマクロライド系薬による産生抑制について検討した。本薬がムチン産生を抑制することをELISA法で確認し、アスペルギルス症の治療の選択肢の一つになることを証明しえた。
