常位胎盤早期剥離の分子マーカーの同定とその臨床的意義に関する研究

Outline of Annual Research Achievements

私どもは、平成26年度までにC19MC領域のmicroRNAのうち、母体血漿中cell-free miR-517c流入量と常位胎盤早期剥離との関連を見出していた。
そこで、平成27年度は、まず、母体血漿中cell-free miR-517c流入量を用いた常位胎盤早期剥離の診断精度について検討した。ROC解析の結果、母体血漿中cell-free miR-517c流入量を用いたAUC値は0.692 (95% confidence interval: 0.519-0.864) であった。母体血漿中cell-free miR-517c流入量15669.6 copies/mLをカットオフ値に設定した場合、その感度ならびに特異度は、73.3% (11/15) ならびに62.5% (15/24)であった。また、陽性適中率ならびに陰性適中率は、それぞれ 55.0% (11/20)ならびに78.9% (15/19)であった。
次いで、母体血漿中cell-free miR-517c流入量と常位胎盤早期剥離の程度との関連について検討した。その結果、剥離の程度が50％以上の群における母体血漿中cell-free miR-517c流入量は、50％未満の群おけるそれと比較して、有意に高値を示した（p値=0.03）。
以上より、母体血漿中cell-free miR-517c流入量は、常位胎盤早期剥離のバイオマーカーであることが示唆された。
