統合失調症と気分障害の連続性：遺伝子、認知、統合失調型パーソナリティの包括的検討

Outline of Annual Research Achievements

近年、大規模ゲノムワイド関連解析をはじめとした実証的研究において、統合失調症と気分障害の類似性が議論されている。そこで本研究では、統合失調症患者、気分障害患者、健常者を対象とし、候補遺伝子の多型、広汎な認知機能、パーソナリティを評価することにより、これら2疾患の連続性を検討した。
平成25年度は、まず、健常成人においてSchizotypal Personality Questionnaireにより評価した統合失調型パーソナリティと認知機能が関連することを明らかにし、統合失調症と健常者の間に統合失調型パーソナリティを介した連続性が存在する可能性を示唆した。次に、双極性障害や統合失調症との関連が示されているANK3遺伝子の一塩基多型rs10761482が双極性障害患者の認知機能に影響を与えることを明らかにし、ANK3遺伝子は認知機能に影響を与えることによって双極性障害の発症に関連する可能性を示唆した。平成26年度は、統合失調症寛解患者、非寛解患者、健常者において、Temperament and Character Inventoryを用いてパーソナリティを評価するとともに、COMT遺伝子のVal158Met多型がパーソナリティに与える影響を調べた。非寛解患者のパーソナリティに比較し、寛解患者のパーソナリティはより適応的なプロフィールを示し、健常者のプロフィールに近いものであった。非寛解患者においてCOMT遺伝子のMetアレルは報酬依存や協調性の低さと関連したのに対し、寛解患者ではVal158Met多型とパーソナリティの間に関連がみられなかった。したがって、この多型は統合失調症における寛解とパーソナリティの関連を修飾する可能性が示唆された。
これらの一連の研究結果は、関連する先行研究の知見を発展させ、統合失調症や気分障害の病因・病態解明の一端となるエビデンスを供するものである。
