口腔上皮内癌における細胞競合現象の認識とその病理学的意義の解明

Research Abstract

口腔粘膜上皮内癌(CIS)および口腔扁平上皮癌(SCC)を含む外科切除材料100例以上の症例を用いて、癌‐非癌部における明瞭な上皮内病変境界を詳細に検討した。癌-非癌部の境界の確認には、ケラチン17陽性癌領域-ケラチン13陽性非癌領域界面を用いた。
その結果、癌-非癌部境界において、癌細胞に高頻度にアポトーシス小体（60%）および好酸性の硝子体形成（36%）が認められ、免疫組織化学ではともにcaspase-3陽性を示し、共通の細胞死機序がかかわっていることが示唆された。またこれらの結果は、界面部での癌細胞におけるアポトーシス性細胞死の亢進を意味し、癌細胞が敗者となることが示唆されたものの、癌-非癌境界部においては細胞競合関連因子の強調は免疫組織化学では認識されなかったため、さらに細胞競合関連因子について検討中である。その他の特徴的な形態変化として、細胞の紡錘形化(64%)および細胞間隙の拡大(71%)が高頻度に観察され、これらは上皮-間葉形質転換（EMT）に類似する現象であり、EMTに関連する分子に関しても今後の検討が必要と考えている。上述の癌-非癌境界に形成される硝子体は、口腔粘膜に発生する扁平苔癬のシバット小体と形態的な共通点が多く、扁平苔癬のシバット小体についても検討を行った。シバット小体は、上皮下肉芽組織の血管破綻像と関連しており、ケラチン17陽性、caspase-3およびヘモグロビン共陽性が明らかになったことから、上皮細胞の赤血球貪食に関連した細胞死であることが示唆された。これらの概念を癌-非癌境界部にも適応し、同様の機序が癌-非癌部界面での細胞死現象にも生じているかを検討し、細胞競合現象との相互的関連性についても検証を進めていく予定である。

Reason

癌-非癌界面を含む組織標本の検討では、界面部において癌細胞の細胞死が亢進するという現象が明らかになり、それらがアポトーシス性の細胞死であることが免疫組織化学から認識され、癌細胞が細胞競合現象の敗者となることが示唆され、当初の仮説が検証されつつある。

Strategy for Future Research Activity

より効率的に、細胞競合因子に関連する候補分子を検討するため、癌-非癌部のタンパク質発現動態を網羅的に検索することを目的に、癌-非癌部組織試料からレーザーマイクロダイセクショクンにより選択的に癌、非癌部を採取し、液体クロマトグラフィータンデム質量分析（LC/MS/MS）を行う予定である。これらの結果をもとに、癌-非癌界面における細胞競合関連因子を検索し、免疫組織化学および培養細胞を用いた試験管内実験に応用する予定である。
