Activation of anti-tumor immune responses by the control of regulatory T cells

Outline of Annual Research Achievements

制御性T細胞により抑制されたがん抗原特異的エフェクターT細胞、特にCD8+T細胞のフェノタイプについて検討を進めた。前年度はとくに自己由来がん抗原に対する免疫抑制について検討したが、本年度はさらに外来抗原に類似したneoがん抗原に対する免疫抑制を検討した。遺伝子変異の蓄積により生じたがん細胞内には、正常細胞が本来もっていない非自己の物質ががん抗原として存在する。これらは、遺伝子変異によりがん細胞で新たに出現したことから免疫系に認識されやすいと考えられ、neoがん抗原とよばれ自己由来抗原と区別される。制御性T細胞により抑制されたneoがん抗原特異的CD8+T細胞は、自己由来がん抗原特異的CD8+T細胞と異なり制御性T細胞存在下においても増殖可能であることが明らかになった。しかし増殖したneoがん抗原特異的CD8+T細胞は、PD-1などのCo-inhibitory分子を強発現しており、抗PD-1抗体投与により細胞増殖、サイトトカイン産生が増強した。これは抗PD-1抗体投与時に自己免疫応答より抗腫瘍免疫応答が優位に誘導されることと一致していた。またこれらのneoがん抗原特異的CD8+T細胞は、転写因子T-betを高発現していることが示された。従来T-betはPD-1発現の抑制分子とかんがえられてきたため、T-betとPD-1発現との関連を検討している。これらのデータに基づき、少数例ではあるが、PD-1治療を受けた患者検体を解析したところ、腫瘍浸潤CD8+T細胞がT-betを高発現している方が、治療効果が良好である可能性が示された。
本研究をもとに、T-bet 発現CD8+T細胞が免疫チェックポイント阻害剤治療のバイオマーカーとなることが示唆され、AMED研究への展開を検討している。
