Long-term effects of major tsunami on the incidence of acute cardiovascular events

Outline of Annual Research Achievements

岩手県沿岸地域と内陸地域を対象として、2011年の大震災津波被害後の2011年, 2012年, 2013年, 2014年に心筋梗塞と突然死の発症率がどのように変化したかを発災前の２年間の平均発症率と比較した。対象地域を津波の被害の大きかった地域（陸前高田、大船渡、山田町、大槌、宮古など）と少なかった地域（岩手県北内陸、久慈、種市）で比較した。心筋梗塞症と突然死の定義はWHO-MONICA基準に従った。対象地域の全病院でカルテ調査を行い心筋梗塞例（病院受診例）および突然死例（救急外来や院外死亡例を死亡小票調査などで調査）を2011年から2014年の４年間実施した。その結果、心筋梗塞症の発症率は対象期間の4年間にわたり津波被害の大小にかかわらず不変であった。しかし、突然死は津波被害の少なかった地域では不変であったのに対し津波被害の大であった地域では発災後4年間にわたり発症率は倍増し、減少する傾向はみられなかった。また、市町村ごとの突然死の発症率の増減オッズ比と各市町村の津波浸水率（市町村内の建物面積のどの程度が津波浸水を被ったの割合）の間に正相関がみられた。この結果（津波被害地での突然死の増加）の理由として、津波被害の大きかった地域住民は心的ストレスに強く晒されており（家族などとの死別、失業、地域コミュティの崩壊）、また、住民が脆弱化（健康で裕福な住民は内陸などに移住し、身体的あるいは経済的な脆弱性を有する住民の割合が増えた）したことが推定された。
