| Outline of Annual Research Achievements |
IgG4関連疾患の病態および治療法については、いまだ不明な点が多い。我々は、Th2優位の免疫反応を自然発症しIgG4関連疾患のモデルマウスであるLAT Y136F変異マウスを用いて、IgG4関連疾患の病態の解明と新規治療法の確立を本研究の目的としている。
本年度は、APRILについて、IgG4関連疾患患者の病変組織におけるIgG4発現とAPRIL発現との関連について解析した。その結果、病変部における強拡大視野あたりのAPRIL発現細胞数とIgG4発現細胞数は正の相関関係の傾向を認めた。
我々はAPRILと相同性を有するB cell activating factor belonging to the tumor necrosis factor family (BAFF)の発現について検討した。まず、ヒトのIgG4関連疾患の唾液腺および腎サンプルを用いてた検討を行った結果、病変部位においてBAFF発現の増加を認めた。さらに各細胞表面マーカーとBAFFの二重染色を行った結果、マクロファージ、M2マクロファージ、T細胞、B細胞に二重陽性細胞を認めた。ステロイド治療前後で病変局所のBAFF発現は減少傾向を認めたが、APRILと異なり治療後組織においても相当の発現の残存を認めた。さらに、4-10週齢のLAT Y136F変異マウスの腎臓におけるBAFFの発現を免疫染色法を用いて検討した結果、炎症部位に一致して、BAFFの発現を認めた。以上の結果から、APRILだけでなくBAFFもIgG4関連疾患の病態形成に深く関与する分子であると考えられた。今後、APRILおよびBAFFが新規治療ターゲットとなり得るか、さらなる検討が必要と考えられた。
