| Outline of Annual Research Achievements |
SCIDマウスによるヒト肺癌細胞の多臓器転移モデルにおけるこれまでの解析から、肺癌細胞株のsphere形成能を有する細胞として純化した幹細胞分画は野生株に比較し肝転移が増加することを確認し、これらの細胞株を用いて肝転移に関連のある遺伝子Bを同定した。まず小細胞肺癌細胞株SBC5に対してレンチウイルスによるshRNAシステムを用いて恒常的に遺伝子Bをノックダウンした細胞株を作成した。通常の遺伝子Bノックダウン株および幹細胞分画であるsphere形成細胞分画の多臓器転移能を検討したところ、いずれの実験においても遺伝子Bのノックダウン株は特異的に肝転移数が減少することを確認した。その後、肺腺癌細胞株であるLC319/bone2に対して同様にノックダウン細胞株を作成し検討を行った。その結果、肝転移は減少したものの、肺転移についても減少する傾向を認めた。研究データの再現性を確認するため、さらにLC319/bone2のsphere形成分画での再実験を予定している。一方で、最近7年間で当科で診療した小細胞肺癌患者を対象に予後因子を検討した。多変量解析の結果、年齢、PS、間質性肺炎、肝胆道系酵素上昇が予後因子として同定された。また肝転移とPSに相関を認めた。以上の結果から、小細胞肺癌患者においては、間質性肺炎の合併とともに、肝転移の重要性が示唆された。臨床的にはさらに大規模な症例を対象とした研究が望まれる。以上の基礎研究および臨床研究成果から、肺癌特に小細胞肺癌においては、肝転移を標的とした治療の重要性が示唆された。
