入院患者の転倒は、身体能力の低下や生活の質に影響を及ぼし、特に高齢者においてはしばしば致命傷となりうる。転倒には、様々な要因が関連していることが判明しているが、行動要因と環境因子の相互の関連は十分に検討されていない。自宅および医療機関の転倒発生率は高齢化に伴い急激に増加することが推察され、転倒起因の外傷件数は2030年には2倍以上になると危惧されている。本研究の目的は、入院患者の転倒に関与する行動要因に及ぼす影響を、季節および日内変動という環境因子に焦点を当て検討した。本研究のデザインは後ろ向き調査研究である。方法は2010年4月から2014年3月までの転倒・転落に関するインシデント・アクシデント記録を後ろ向きに調査し、その時の行動要因と当時の日の出時刻、夜の長さ、気温を含む季節および日内変動の環境因子を気象庁の記録から調べ、それらの環境条件との関連を調査した。その結果、3,037件のインシデント・アクシデント記録のうち、転倒に関するものは464件であった。本調査における平均転倒率は1.4±0.5/1,000（占有床日数）であった。季節および日内変動について転倒率を比較した結果、10月から2月の夜明けの転倒率が4月から9月の転倒率より高かった。また、転倒に関連する行動要因で最も頻繁なものは排泄行動であり（56.9％, n= 264）、転倒の57.1％は夜間に発生していた。多変量解析の結果では、夜の長さが夜間転倒率の増加に有意に関連していることが示された（p=0.047）。入院患者の転倒は、11月から3月の早朝に増加し、排泄行動に関連する転倒の傾向があることが示唆された。これらの結果より、特に転倒リスクの高い時間と季節に、排泄行動に関連した細心の注意を払ったケアを提供することが、転倒の発生率を低下させる一助になると考える。
