Outline of Final Research Achievements

In order to provide high-quality discharge planning, it is necessary to develop a team approach with interprofessional work and evaluate the support. Therefore, we conducted a literature review, an interview survey with professionals (doctors, physiotherapists, occupational therapists, speech therapists, nurses, care workers, social workers) working in the convalescent rehabilitation ward, and interviews with patients and their families discharged from the ward, and made a draft of a quality evaluation index for discharge planning in the convalescent rehabilitation ward. After that, in order to confirm the stability of the draft and to examine the content validity, a questionnaire survey was conducted on the professionals who obtained the cooperation in the interview survey, and the items were refined. As a result, it became a draft index that consists of a total of 118 items: 27 structural indicators, 62 process indicators, and 29 outcome indicators.

Academic Significance and Societal Importance of the Research Achievements

これまで退院支援の評価は看護師の認識や実践により評価されることが多く、患者や家族の視点、医療提供者の視点、資源効率やコストといった社会的福利からの視点など、多方面からの包括的評価方法の確立が必要であった。今回作成した指標案は、多職種や患者・家族の視点を取り入れており、退院支援を包括的に評価することが可能である。また、ドナベディアンモデルを用いて指標案を作成したことにより、各指標項目が推奨事項として活用できることから、支援の質改善にもつながることが期待される。
