Research Abstract |
The auditory brain stem response (ABR) was recorded in 357 cases of comatose patients. The caloric test was examined in 92 cases of them and Encephalograms (EEG) were observed in 112 cases. The responses of the caloric vestibular test were also classified into four types, i.e. normal response, nystagmoid reaction (slow deviation with slow return), eye deviation (conjugate ocular deviation without returning), and no response. Of the 201 cases in which the inter wave latency (I-V)of initial ABR was normal, 109 cases recovered and 92 casesdied. As for the 104 cases which showed only I wave or noresponse, they all died. Concerning the relationship between the results of final measurement and prognosis, 41 died out of the 152 cases in which the inter wave latency (I-V) was normal. As for the 52 cases which showed wave I to wave V early but eventually shifted to no response, they all died. This suggests that ABR is useful in observing minute changes of the brain stem and directly reflects the function of the brain stem. The EEG was active in 38 cases and became isoelectric in 74 cases. As for ABR of the cases in which EEG was active, waveforms of wave I to wave V were recognized in all the cases. As for early ABR of the cases in which EEG became 73 cases isoelectric, only wave I or no response was observed in 40 cases, and waveforms from wave I to wave V noted in 31 cases. The interwave latency (I-V) was within the normal range in 28 cases. However, cases showing the waveforms up to wave V became non-responsive with the lapse of time and died. These findings suggest that combination of EEG and ABR enables one to learn more exactly the central functioning those with disturbance of consciousness.
