Outline of Research at the Start

研究代表者は神経障害性モデルマウスに抗炎症性M2ミクログリア誘導因子を投与することで、組織破壊的な炎症反応を抗炎症・組織再生型に転換し疼痛制御を促進することを見出した。また誘導されたM2ミクログリアの放出する細胞小胞体(エクソソーム)が周囲のSchwann細胞に取り込まれ炎症状態の制御や細胞死の抑制を行うことで疼痛制御を行っていた。本研究は、ミクログリア由来エクソソームに含まれる複数のmiRNAから疼痛制御に関与する共通標的遺伝子を同定する。またM2ミクログリア由来エクソソームの投与が、同定された遺伝子へどのように影響を及ぼすか検証する。

Outline of Final Research Achievements

In neuropathic pain, injury or diseases of the somatosensory system often result in chronic pain. Currently, there is no effective drug for the definitive treatment of NP. We investigated the therapeutic potential of conditioned medium derived from stem cells from human exfoliated deciduous teeth (CM) against NP using a mouse partial sciatic nerve ligation (PSL) model. In the behavioral test, intravenous administration of CM greatly improved PSL-induced hypersensitivity. We found that treatment with CM resulted in the recruitment of M2 macrophages in the injured sciatic nerve and ipsilateral L4/L5 dorsal root ganglion and suppressed microglial activation in the spinal cord. We found that M2-CM directly suppressed the expression of nociceptive receptors as well as proinflammatory mediators in Schwann cells.

Academic Significance and Societal Importance of the Research Achievements

神経障害性疼痛などの慢性疼痛は発症機序が不明な点から、根本的な治療法の確立には未だ至っていない。近年、神経組織の炎症が慢性疼痛の維持と悪化と関連すると報告がある。本研究は、PSL後に末梢から脊髄で活性化したマクロファージ/ミクログリアをM2へ誘導することで、マウスの疼痛過敏状態を改善することを見出した。さらにM2から分泌される疼痛制御因子群を明らかにできた。これらの知見は、神経障害性疼痛だけなく、多くの慢性疼痛へ対する新たな製剤化および創薬への途を開いた。以上の方法は、慢性疼痛の原因となる神経炎症と免疫細胞をターゲットにしているため、既存薬とに比べ有効であると考える。
