変形性関節症(OA)の軟骨変性進行を抑制できる有効な治療法は、未だ確立されていない。申請者の先行研究において、C10orf10/Decidual protein induced by progesterone(DEPP)という新規因子がミトコンドリアに対する選択的オートファジーであるマイトファジーを活性化させ、軟骨細胞の生存維持し、軟骨変性を抑制することを明らかにした。この先行研究を発展させ、本研究では、OA治療薬開発を前提として、OA病期を考慮したC10orf10/DEPPの詳細なマイトファジー制御機構の分子メカニズムを明らかにすることを目的とする。

| Outline of Final Research Achievements |
The aim of this study was to elucidate the molecular mechanism of C10orf10/decidual protein induced by progesterone (DEPP) and to develop disease-modifying therapies for osteoarthritis (OA).
C10orf10/DEPP was not found to be expressed specifically in the OA stage. We selected compounds that induce C10orf10/DEPP, and sertraline, a selective serotonin re-uptake inhibitor, was selected as a candidate. Stimulation of human chondrocytes with 1-10 μM sertraline for 24 hours resulted in a more than 2-fold increase in C10orf10/DEPP gene expression compared to controls.

| Academic Significance and Societal Importance of the Research Achievements |
変形性関節症（OA）は、進行性の軟骨変性により関節機能を著しく障害する筋骨格系領域における代表的な加齢性疾患であり、要介護の原因疾患の第５位で高齢社会における我々の健康寿命に大きな悪影響を与えている。末期症例に対しては金属製の人工関節置換術が良好な成績を上げているが、初期および進行期OAに対する疾患修飾治療薬は未だに存在しない。今回の成果によって、セルトラリンが候補薬として選定された。セルトラリンは精神科領域で臨床使用されている薬剤であり、OAに対する治療効果を認めれば、ドラッグ・リポジショニングにより臨床応用しやすい利点がある。
