研究概要

今年度の海外学術調査研究はシリア、ヨルダン、モンゴル、そしてポルトガルの4か国で行なった。
シリアおよびヨルダンではベーチェット病患者が多数みられ、眼科、リウマチ科ではベーチェット病が重症例の大半を占めていた。しかし原田病はみられず、日本とは異なっていた。シリアではまだこれら難治性のぶどう膜炎を集中的に治療する体制はできておらず、この点について今後の研究協力をすすめるために助言を行なった。これにたいしヨルダンではベーチェット病専門病院が整い、50名を越えるベーチェット病典型例を診察することができた。HLA-DNAタイピンクのための採血もスムーズにでき、分子遺伝学的検素材料を持ち帰ることができた。
一方、モンゴルでは日本とほぼ同様の難治性ぶどう膜炎患者がみられ、典型的なベーチェット病、原田病が診断できた。しかしサルコイドーシスはみられなかった。今回の調査により、今後の共同研究のための基礎が確立された。
シルクロードの西端に位置するポルトガルには、他の地中海沿岸諸国と同様に多くのベーチェット病が見出された。しかし原田病やサルコイドーシスは多発傾向がみられなかった。今回の調査により、ポルトガルのベーチェット病患者も我々が日本人患者で見出したHLA-B51との相関を有することが明らかにされた。次回の共同調査時には、HLA-B*5101との相関の有無を調べる必要がある。
このほか、今年度はこれまでに収集したサウジアラビアやギリシャでのぶどう膜炎患者の分子遺伝学的解析をすすめ、人種を越えたHLAクラスIとの同一相関を見出した。
