研究成果の概要

肝硬変患者では病態の進行とともに糖代謝異常を来すことが知られている。肝予備能の低下や肝血流の低下に伴う糖取り込みの低下，糖新生の低下などが原因と推察されている。一方、血中インスリンおよびグルカゴン濃度が上昇していることなどから、膵臓での分泌異常の関与が示唆されているが、その機序は明らかではない。そこで、肝硬変における膵血流の異常が膵臓での分泌異常に及ぼす影響を明らかにすることを目的として検討を行った。その結果、造影超音波検査法による膵血流評価が有用であり、その手法を用いて評価すると肝硬変では膵うっ血がみられ、インスリン分泌とも相関していることを明らかにした。
