歯牙疼痛の内因性ペプチドによる制御に関する研究

研究概要

多種類のペプチドが中枢神経系に分布することが明らかにされてきたが、中でも特にサブスタンスP(SP)などは一次求心線維に高濃度に分布し、痛覚伝導に深いかかわりをもつとされる。また、エンケファリン(EK)などのオピオイドペプチドが疼痛の制御に重要な役割を果たしている。本研究では歯髄疼痛伝導路におけるこれらペプチドの動態とその生理的役割に重点をおいて行われた。
(当該年度に得られた結果
1)歯髄組織のホモジネート、マイクロゾーム画分、あるいは上清に含まれるEK前駆体を酸性抽出してセファデックスG-100ゲル濾過で精製すると分子量58、000を持つことがわかった。歯髄EK産生酵素はライソゾーム画分に多く含まれる。CMセファデックスC-50で精製すると(400倍)、23.6kDaの分子量をもち、カテプシンBのそれに近い。ラット歯髄培養線維芽細胞でもBK,des-Arg^8BK、あるいはLーアルギニンがカテプシンBを活性化しメチオニンEKが産生される。
2)モルモット三叉神経節膜分画にはL型とN型のカルシウムチャネルが存在し、モルヒネはN型のチャネルに影響を及ぼす。
3)ウサギ三叉神経脊髄路核尾側亜核浅層領域をPush-pullカニューレを用いて潅流すると、歯髄刺激によりSPとセロトニン(5-HT)の放出がみられる。モルヒネは5-HTの放出を高めると同時にSPの放出を抑える。大縫線核の刺激、および5-HTの局所適用によりSPの放出が抑られることから、下降性の5-HTニュウロンの活性化が疼痛制御に関与することが明らかになった。このSPの放出は鍼刺激によっても抑制された。
