α線発癌の遺伝子解明を最終目的とするトロトラスト被注入者の総合研究

研究概要

町並はトロトラスト被注入者に発生した肝細胞癌21例、胆管細胞癌37例、血管肉腫30例を用いp53遺伝子のexon5,6,7,8における変異を検索したところ、点突然変異が肝細胞癌11例中7例(63.6%)、胆管細胞癌5例中1例(20.0%)、血管肉腫3例中0例(0%)にみられ、肝細胞癌にかなり高い頻度で点突然変異が認められた。一方、p53遺伝子のLOHは肝細胞癌9例中3例(33.3%)にみられ、p53の点突然変異の頻度よりもLOHの頻度が低く、LOHの認められない症例でも4例に点突然変異が認められた。
森はトロトラスト被注入剖検例383例の統計学的解析により、1998年7月15日現在で被注入者群の生存率が6.9%まで低下しており、対照者群の32.2%に比して、有意に低下していることを明らかにした。
村松は高周波誘導結合プラズマー質量分析法(ICP-MS法)を用い、トロトラスト被注入者の種々の臓器中のトリウムの分析を行い、脾臓(平均値:21,400μg/g)、肝臓(平均値:2,100μg/g)、骨髄(平均値:600μg/g)の順に沈着量の多いことを明らかにした。
石川は2匹のサルにトロトラストを注入3〜4年後に肝臓、脾臓及び骨髄のトリウムの量を測定し、沈着量の比率が54:6:41及び75:4:21という結果を得た。即ち、肝、骨髄そして脾の順に沈着量の多いことが明らかとなった。
福本はトロトラスト被注入者に発生した胆管細胞癌における癌遺伝子rasと癌抑制遺伝子p53の変異を検索し、14%の症例の癌部にコドン12G→A transitionを認めた。またのエクソン5-8の解析を行い、22例中4例(18%)で点突然変異(75%はA→G transition)を認めたが、2例では非癌部に変異が見られた。
その他、志賀はトロトラスト被注入者における腫瘍発生を観察する場合の参考として、Ir^<192>針をラットの肝に刺入し、約600日後に肝の組織学的検索を行い、腫瘍様の結節を認めた。
