生体肝移植後の拒絶反応の新しい診断法の開発に関する研究

研究概要

東京大学医学部附属病院では平成8年1月より生体肝移植を開始し平成11年2月までに52例を施行した。Recipientの年齢は平均14.9歳(8ヶ月〜62歳)であり、男性24例、女性28例であった。原疾患は胆道閉鎖症35例、原発性胆汁鬱滞性肝硬変5例、肝硬変5例、Alagille症候群2例、その他5例であった。レシピエント52例中50例が生存中である。免疫抑制療法はFK506+ステロイドを基本としているが、状況に応じてCyAを使用している。移植後順調にT-BIL,GOT,GPT,ALP,G-GTPが低下している過程で、T-BIL、肝酵素、胆道系酵素の急激な再上昇が見られた時、急性拒絶と診断し治療を開始するが、必要あれば肝生検を施行し病理学的に急性拒絶を診断した。このような診断基準のもと52例中15例で急性拒絶が診断されたが、残り37例では急性拒絶は起こらなかった。肝生検では肝内グリソンの著明な細胞浸潤、胆管破壊、門脈の内膜炎が認められた。サイトカイン(TNF-a,IL-2,IL-4,soluble IL-2 receptor,IL-6,IL-10)は5例で測定できたが、急性拒絶時にsoluble IL-2 receptorのわずかな上昇がみられたが、有意な指標となるものはなかった。一方、末梢血液中の好酸球数、肝機能障害と急性拒絶の関係を33例で検討したところ肝機能障害のみられた29例中好酸球数増加がみられたのは23例で急性拒絶がみられた7例は23例に含まれていた。従って、末梢血液中の好酸球数増加と肝機能障害は急性拒絶を示唆する有力な情報であり、末梢血液中の好酸球数増加のない肝機能障害では急性拒絶の可能性が少ないと考えられた。以上より急性拒絶の診断は末梢血中のT-BIL,GOT,GPT,ALP,G-GTPの上昇、それに伴う好酸球数増加が有用な指標となり、現時点で測定可能な様々なサイトカインの変動は有効な指標とならないと考えられた。
