ヒトTCR/HLA/ペプチド複合体の相互作用ならびに自己免疫疾患の病因解析

研究概要	Vogt-小柳-原田病(VKH)はメラニン細胞を含む組織に障害が生じる疾患であり、臨床症状として虹彩毛様体炎、漿液性網膜剥離、皮膚白斑、無菌性髄膜炎、難聴等を示す。また、その疾患感受性はHLA-DR4(DRB1*0405-DR53-DQ4ハプロタイプと強い相関を示す。VKHはメラニン細胞を標的とする自己免疫疾患と考えられているが、その抗原蛋白は未だ同定されていない。我々はVKHにおける標的自己抗原を同定し、その病因解明に資することを目的として、患者血清中の自己抗体に反応するウシぶどう膜由来のcDNA発現ライブラリーの探索(SEREX法)を行った。その結果、標的抗原の候補としてLens epithelial cell derived growth factor (LEDGF)、および、本研究においてUACA(uveal autoantigen with coiled coil domains and ankyrin repeats)と命名した新規蛋白質を同定した。LEDGFのGST融合蛋白を用いて、ELISA法によりVKH、汎ぶどう膜炎を併発したベーチェット病(BD)とサルコイドーシスの患者群と健康対照群における抗LEDGF自己抗体価を測定した。抗体陽性率は、VKH66.7%、BD34.4%、サルコイドーシス25.0%、健康対照群21.6%であった。VKHにおける抗体陽性率は、健康対照群と比較して有意に高かった(p<0.001)。また、UACAのC末端部分(261アミノ酸)をGST融合蛋白として作成し、抗UACA抗体価を測定した。抗体陽性率は、VKH19.6%、BD28.1%、サルコイドーシス21.1%、健康対照群0%であった。抗体陽性率は健康対照群と比較して、VKH(p<0.005)、BD(p<0.001)、サルコイドーシス(p<0.05)において統計学的に有意に高かった。今後、自己抗原を認識するT細胞の有無ならびに同定した自己抗原を実験動物に免疫して、自己免疫性ぶどう膜炎を誘導できるか否か検討する必要がある。
