ストレスホルモン感受性マーカーによるアトピー素因のin vitro診断法

研究概要	「ストレスホルモンの感受性マーカーによるアトピー素因のin vitro診断法」の開発研究の一環として、60名の成人男性を被験者とし、質問紙(GHQ28)でストレスにともなう精神身体の自覚症状の有無を評価し、同時に末梢血を採取し、総IgE濃度およびサイトカイン類(IL4、IL10、IL12、IFN-γ)の産生能を測定した。GHQ28のスコア8点以上を高ストレス、8点未満を低ストレスとし、また総IgEが100IU/ml以上をアトピー、それ未満を非アトピーとしてサイトカイン類の産生との関連を解析した。その結果、LPS刺激によるIL10とIL12の濃度の比率IL12/IL10はアトピーでかつ高ストレスである群では他の群に比べて低かった。IL12/IL10比が低いことはIFN-γ産生が低下する可能性を示唆し、実際この群ではIFN-γ/IL4比も他の群に比べて最も低かった。以上の結果はアトピー素因にストレスが加わるとさらにTh1/Th2バランス(IFN-γ/IL4比)がさらにTh2側に傾きアトピー素因が強化されることを意味する。ストレスによりアトピー性疾患が増悪ことが臨床的に経験されるが、IL12/IL10比の低下がストレスによるアトピー性疾患増悪メカニズムのひとつとなっていることが示唆された。現在は同一の被験者60名を対象にしてサイトカイン産生におよぼすストレスホルモン類(コルチゾル、アドレナリン)の影響の検討、さらにそれらに関連する遺伝子を解析すべく研究を進行中である。
