悪性脳腫瘍髄腔内播種の機構解明の研究

研究概要	1.悪性グリオーマにおけるアポトーシス関連分子の発現と放射線化学療法感受性:1987-2000年までに一律の治療(可及的摘出、放射線療法および化学療法ACNU±IFN-β)を行った退形成星細胞腫(AA)29例、退形成乏突起膠腫(AO)9例および退形成乏突起星細胞腫(AOA)17例を対象とした。各分子の免疫組織化学による発現を10%をcutoff pointとして評価し、組織型および治療反応性との相関を検討した。またAA 8例、AO 5例およびAOA 10例においてはイーストを用いたp53のfunctional assayも行った。組織型と治療感受性をみるとAO 5/6、AOA10/11に反応が見られたのに対して、AAでは8/26と組織型間に有意差がみられた。免疫組織化学におけるp53の陽性率はAA 7/29、AO 0/9、AOA 0/17で組織群間で有意差があった。Bc1-2陽性率はAA 7/29、AO 1/9、AOA 5/17で有意差はなかった。Bax陽性率はAA 14/29、AO 9/9、AOA 15/17で有意差がみられた。治療に対する反応とBc1-2とBaxの発現パターンをみると-/+の17/19に反応が見られたのに対して-/-および+/+ではそれぞれ1/13および4/10に反応が見られたのみで、発現パターンと治療反応に有意差がみられた。p53 functional assayではmutationがみられたのはAO 0/5、AOA 1/10であったのに対してAAでは8/8と組織型間に有意差がみられた。以上のことよりAOおよびAOAはAAに比較して有意に放射線化学療法の感受性が高く、それは治療によりもたらされたDNA傷害からアポトーシスに至る機構が関与していることが示唆された。 2.悪性グリオーマ髄腔内播種モデルの作成および治療:ラットグリオーマ細胞株C6を体重150gのラットの大槽内に10^6個/0.1ml接種して髄腔内播種モデルを作成し、2日後にACNUあるいはリポソーム包埋ACNU1.5mg/kgを大槽内に投与し、対照群との生存期間を比較した。リポソームの構成脂質にはホスファチジルコリンを用い、逆相蒸発法とpH勾配法を用いてリポソーム包埋ACNUを作製した。これらの治療による生存期間中央値は対照群(未治療群)20日、ACNU投与群23日、リポソーム包埋ACNU投与群30日と治療効果を認めた。
