変形性関節症の病態形成におけるインシュリン様成長因子とその結合蛋白の役割

研究概要

変形性関節症(OA)では軟骨基質の分解が進み関節軟骨は変性していく反面、骨棘形成や軟骨下骨の肥大などの増殖性変化もともない、その病態は複雑である。一方、インシュリン様成長因子(IGF)は骨、軟骨の増殖や分化の促進因子であり、正常な軟骨の代謝機能維持に重要であるが、OAなどの病的な状態ではその作用機序に何らかの異常が生じていることが考えれる。IGFの局所における調節因子としてIGF-結合蛋白(IGFBP)が重要な役割をもっていることが推測される。われわれはこの点をさらに解明するために、OA軟骨におけるIGFBPの発現パターンを蛋白、及び遺伝子レベルで観察し正常組織と比較することを今回の研究目的とした。まずOAのX線学的評価を術前に行ない、インフォームドコンセントに基づいて、手術時に摘出した骨頭の一部より関節軟骨を採取した。また軟骨の一部をトリプシン、およびコラゲナーゼで酵素消化し、遊離した細胞を10%牛胎児血清含有培地で培養し、産生されるIGFBPをSDS-ポリアクリルアミドゲル電気泳動後、Western-ligand blotting法で検出し、Immuno-blotting法で同定した。OA軟骨ではIGFBP-3とIGFBP-4の産生が主であったが、その他にも30KD付近に2つの少量バンドがみられた。関節軟骨組織を光顕で観察後、IGFBP-3.-4-,-5の抗体で免疫染色をおこない軟骨における分布を調べ、正常軟骨と比較した。OAではこれらすべてのIGFBPの発現が増加していることがわかった。(以上の成果の一部は日本整形外科基礎学会で発表した。)
