悪性腫瘍で高発現しているWT1に対する細胞性および液性免疫反応の解析―特に癌ワクチンの開発に向けて

Research Abstract |
白血病の患者では,高頻度に,IgM型及びIgG型のWT1抗体が検出され,白血病が完全寛解(CR)になると,WT1抗体も消失することが明らかになった(Blood, 2002. これは,CRになって,白血病細胞が減少ないし消失し,WT1タンパクによる抗原刺激がなくなったためと考えられる)。さらに,前白血病状態である骨髄異形成症候群(MDS)では,RAから,RAEBへ,さらに,RAEB-tへと病期が進行し白血病に近づくにつれ,WT1抗体が、IgM型からIgG型にクラス・スイッチすることが明らかになり,WT1発現腫瘍細胞をもつ白血病やMDSの患者では,WT1タンパクに対するhumoral immune responseが起こりうることが明らかになった(Blood, 2002)。このWT1タンパクに対するhumoral immune responseの惹起は,WT1タンパクに対するhelper T cell responseの惹起を意味し、これらの患者は、WT1タンパクに対するhumoral immune responseのみならず,WT1タンパクに対するT cell immune responseも充分に惹起できることが明らかになり,WT1タンパクに対するCTL response(キラーT細胞の誘導)も惹起しうる可能性が強く示唆された。そこで,WT1ペプチドを用いたがんの免疫療法の第I相臨床試験をはじめた。WT1ペプチド0.3mgを2名のMDS患者に皮内注射したところ,WT1特異的細胞傷害性T細胞が増加し,白血病細胞が著減し,WT1ペプチドを用いたがんの免疫療法が有効であることが実証された。
