アルドステロン合成細胞(APC)を探して：高血圧症臓器障害の新たな機序の解明

Outline of Annual Research Achievements

原発性アルドステロン症の臨床診断にて手術摘除された副腎組織の中で、腫瘍性病変が認められない症例に対して、パラフィン包埋組織より連続切片を作製し、HE染色による形態学的評価、アルドステロン合成酵素(CYP11B2)などのステロイド合成酵素の発現パターンの解析を行った。これら非腫瘍性病変の病理組織診断は通常のHE染色のみでは困難であるが、CYP11B2陽性細胞の球状層における発現動態がびまん性に分布するか、局所に結節状に分布するかにより、大きく2つの病型に分布する事を今回初めて示す事が出来た。また、これら症例の連続切片よりDNAを抽出し 次世代シークエンサーを用いて、近年、アルドステロン過剰産生に関与すると報告されている各種イオンチャネルをコードする遺伝子（KCNJ5、ATP1A1、ATP2B3、CACNA1Dなど）の体細胞変異の有無を解析した。
その結果、CYP11B2陽性細胞が球状層局所に結節状に分布している症例の一部において、今まで APAでのみ報告されていたカリウムチャネルをコードするKCNJ5遺伝子の体細胞変異を新たに発見する事が出来た。この結果からAPAの起源となり得る病変の可能性を初めて今回の検討で示す事が出来た。更にAPAの発生には塩分摂取や局所アルドステロン濃度といった環境因子が強く影響している可能性が示唆されており、これら環境因子によって誘導される酵素やタンパクの発現量についても検討を加えると同時にアルドステロン合成に関与する遺伝子の発現を制御するmicroRNA等のepigeneticな要因についてもこれから検討を更に加える予定である。

Reason

Ⅰ.剖検で得られた正常副腎及び手術検体で得られたAPA症例でAPCを同定する。: 剖検例において、APCは約30%で確認される事を初めて証明した。更にこれらAPCは加齢と共に増加する事も初めて発見し、現在peer review 誌に投稿中である。
Ⅱ.これらAPCにおけるアルドステロン合成動態の詳細をその制御機構を含めて明らかにする。：APCおよびAPAでのカルシウムチャネル関係のタンパク質発現動態との比較検討を行った。また、アルドステロン過剰産生に関与する遺伝子の体細胞変異の頻度をAPCおよびAPA、さらにはAPAの前駆病変となり得る病変についてもこれらの遺伝子の体細胞変異の頻度を比較検討した。引き続き培養細胞でアルドステロン合成と関係がある可能性があるmicroRNAを現在同定しているところである。
Ⅲ.正常副腎皮質、APAでのAPCの病態意義を種々の臨床病理学的因子と相関させて解明する。：正常副腎皮質では加齢との相関を示し、血圧とは有意の相関はない事を認めた。高血圧症例と非高血圧症例におけるAPCでアルドステロン過剰産生に関係する遺伝子の体細胞変異の頻度が異なる事が示されたと同時にAPCの個数や皮質に対する面積比が異なる事についても証明した。これらのAPCの一つがAPAの前駆病変となり得るかについて、epigeneticな要因や食塩摂取等の環境因子によって誘導されるタンパク質や酵素の発現について現在評価中である。
Ⅳ.得られた結果が高血圧症やPAの新しい診断・治療方法の開発に結びつけられる可能性を模索する。：摘出された副腎組織においてCYP11B2の免疫組織化学は病型分類に非常に重要であり、術後の治療法の選択にも関与する事が示された。引き続き術前診断に有用なバイオマーカーの探索を行う予定である。

Strategy for Future Research Activity

1.　正常副腎皮質球状層でアルドステロンを合成している細胞とそうでない細胞間で、形態学的な差異があるのかを電子顕微鏡を用いた超微形態学的観察により検討する。また、KCNJ5陽性のAPAは形態学的に淡明な膨化した細胞質を有する細胞により構成される事が知られており、この細胞の超微形態学的観察により、副腎皮質の球状層に近い細胞なのか束状層に近い細胞なのかを比較検討し、APAの発生母地に迫る。
2. 食塩過剰摂取により誘導される酵素やタンパク質の発現量の差異を原発性アルドステロン症の副腎組織および正常副腎組織において比較する。また、培養細胞を用いてこれらの遺伝子の発現量とアルドステロン合成に関与する酵素の発現量との相関性を検討する。
3. 2.の遺伝子の発現を制御する microRNAやカルシウムチャネルやカリウムチャネルをコードする遺伝子の発現を制御するmicroRNA が発現しているのかどうかを、APA やCYP11B2陽性のアルドステロン合成細胞、陰性の球状層細胞を顕微鏡下で単離し検討する。
4. 2.や3.で同定された遺伝子や microRNAの発現を培養細胞において減弱させ、アルドステロン合成に関与する酵素の発現量の推移を解析する。
