脱髄性神経損傷に起因する難治性神経因性疼痛の治療標的分子の同定

Research Abstract

本研究は難治性の神経因性疼痛の分子基盤を解明するため、神経因性疼痛の原因分子として発見したリゾホスファチジン酸(LPA)による脱髄とそれに伴う可塑的変調機構を明らかにすることを主題としている。本年度は、神経損傷短時間後にLPAがde novo合成されることにっいて詳細な研究を行った。特異的抗体が入手困難なLPAを定量的に測定するためにMALDI-TOF-MAS装置を用いて、nano-structure assisted laser desorption/ionization(NALDI)-TOF-MS解析の確立を行った。LPA1受容体発現細胞におけるバイオアッセイとの比較を行い、またLPA1拮抗薬を用いた薬理学的研究を経て、坐骨神経傷害後約1-2時間後をピークとして脊髄後角と後根にLPC産生が生じることを明らかにし、オートタキシンによりさらに代謝されてLPAとなり、異常痛を発揮することが明らかになった。さらに特徴的な事実としてMass-imagingにも成功し、脊髄後角におけるLPC産生の可視化に成功した。ついで、LPAによる脱髄機としてミエリンタンパク質の分解機構について解明し、その阻害剤による神経因性疼痛抑制効果も確認した。WGAと蛍光蛋白質の融合蛋白質をcre-1oxPシステムにより発現するトランスジェニックマウスを用いて、レンチウイルスによる知覚神経末端部へのCre遺伝子の導入により、後根神経節細胞と脊髄後角神経細胞における蛍光シグナルの確認に成功した。中枢性の慢性疼痛については脊髄損傷、視床痛、冷温繰り返しストレスモデルを確立した。特に後者においては既存治療薬の有効性を動物実験で初めて証明した。予備検討であるが、LPA1受容体シグナルが関与することも明らかにできた。脳における責任部位の特定もでき、末梢性と中枢性疼痛の共通点が明らかになった。
