Outline of Annual Research Achievements

1) 前年度に続いて、症例収集と疫学調査をおこなった。口腔扁平上皮癌と上皮内癌ならびにその前癌病変症例を、中国では上海交通大学、四川大学、ミャンマーでは国立医学研究所、台湾では国立台湾大学歯学部、インドネシアのジェンバー大学、スマトラ・ウタラ大学、バイチュラマ大学歯学部にも抽出してもらい、症例収集をさらに拡充した。それらの症例については、組織標本のバーチャルスライドを蓄積し、コンピュータネットワークによる相互観察討議ができるよう環境整備した。
2)今年度は研究代表者程ならびに分担者朔がインドネシアのメダン市とパダン市を訪問し、ジェンバー大学のMei Syafriadi教授をはじめ、スマトラ・ウタラ大学のAmeta Primasari Tarigan准教授、バイチュラマ大学のDewi Elianora副学部長ならびに現地の口腔病理医と共に症例を相互に検討して申請者らの確立してきた口腔表在性癌の疾患概念と診断基準について討議した。また、中国成都市の四川大学を訪問し、研究協力者の呉蘭雁教授らと口腔表在性癌症例について診断基準の均霑化をはかった。とくに乳頭型の組織像について今後の検討が必要だという共通認識を得た。
3) 新たな病理組織学的診断基準による収集した口腔粘膜表在性癌症例について、口腔癌内相対的発生頻度、これら患者の口腔衛生・感染・全身栄養状態とその地域間の異同と相関を解析した。同時に、各地で収集した症例のパラフィン切片について、keratin (K) 10、K13、 K17、 K19の発現動態と細胞－細胞間接着分子cloudin-1、zonula occludens-1、細胞増殖因子SOX9の発現レベルを評価し、HPVとHHV-1感染も重ねて組織レベルではin-situ組織化学によって確認した。以上の成果の一部については国際学会ならびに国際誌にて発表した。
